We assure you that we will repair or replace any of our dental crowns, porcelain veneers, or onlays for any reason (including accidental trauma) for up to five years provided that:

1) Repair or replacement is done in our office.

2) The tooth in question can be saved.

3) You have had at least two dental cleanings per year since the placement of the crown, porcelain veneers or onlay.

4) You inform us immediately of any dental problem related to our treatment or discovered at your two annual cleanings.

NOTE: If any further treatment is needed at the time of repair or replacement (such as root canal, implant or extraction), an additional fee will be charged.
