Our membership plan is $495 per patient and cannot be combined with dental insurance.
Adult Complete Membership
- 2 routine cleanings, 2 examinations, and any necessary x-rays
- Emergency examination and any needed periapical or bitewing x-rays
- Oral hygiene detailed review and one-on-one guidance
- Oral cancer screening
- Intra-oral photographs
- 15% off all routine restorative treatment, including veneers and implants.
- 15% off of all periodontal treatment, including scaling and root planing and periodontal maintenance cleanings
Child Complete Membership
- 2 routine cleanings, 2 examinations, and any necessary x-rays
- Fluoride varnish application at each cleaning
- Intra-oral photographs
- Emergency examination and any needed x-rays
- Hygiene education
- 15% off all routine restorative treatment
Exclusions & Limitations
- Cannot be used for services or injuries covered under workman’s compensation or automobile injuries.
- Cannot be used for products such as electric toothbrushes, waterpiks, and whitening products.
- All fees are non-refundable
