Here’s the truth. There’s no upper age limit for All-on-4 dental implants. We’ve placed them for patients in their 70s, 80s, and even 90s. What matters isn’t your age on a birth certificate. It’s your overall health and your jawbone structure.

“Age alone doesn’t determine candidacy,” Dr. Doroshenko says. “I’ve treated patients in their 80s who were healthier than some patients in their 50s. What I look at is healing capacity, bone density, and overall health. Not the number of birthdays.”

- Bone Density and Volume: All-on-4 was designed specifically for patients with bone loss. The angled implant placement lets us use the bone you still have, even if it’s limited. Many patients told they need bone grafting at other practices qualify for All-on-4 without it. Dr. Doroshenko uses 3D CBCT imaging to measure your bone precisely. He can see exactly where implants can be placed for maximum stability.
- Overall Health: Your body needs to heal after surgery. Certain conditions can affect healing. Uncontrolled diabetes, heart disease, and autoimmune disorders are concerns. But controlled conditions usually aren’t a barrier. “We work with your physician if needed,” Dr. Doroshenko says. “In most cases, we can proceed safely with careful planning and monitoring.”
- Medications: Some medications affect bone healing. Bisphosphonates for osteoporosis are a common concern. Dr. Doroshenko reviews your medication list carefully. In many cases, treatment is still possible with precautions.
- Gum Health: We need to treat active gum disease before implant placement. This applies to patients of any age. Once your gums are healthy, we can proceed.
- Smoking: Smoking affects healing and implant success. If you smoke, we’ll discuss cessation strategies. Many patients can still receive implants with guidance and support.
- Commitment to Aftercare: Implants need care. You’ll need to brush, clean around the implants, and attend regular dental visits. If you’re willing to maintain your new smile, you’re likely a good candidate.

This is the most common concern we hear, but it’s outdated advice. All-on-4 was specifically designed for patients with bone loss. The angled implants let us work with what you have.
“I’m afraid of surgery.”
