You did it! You went through the surgery and walked out with a new smile. You can eat, speak, and laugh with confidence again. Now what?
All-on-4 implants in Baltimore are an investment. Like any investment, they need protection. What you do in the days, months, and years after surgery determines whether your new teeth last 10 or 30 years.
At Cosmetic & Advanced Dentistry in Owings Mills, Dr. Andrey Doroshenko and Dr. Ed Lazer have helped countless patients restore their smiles with All-on-4. They’ve also seen what happens when patients neglect aftercare. They want you to know exactly what to do to protect your investment. Let’s walk through the aftercare roadmap.
The First 48 Hours: Critical Healing Time
The first two days after surgery set the stage for everything that follows. Your body forms blood clots, reduces inflammation, and begins the healing process.
What to Expect
Some swelling is normal. It usually peaks around 48 to 72 hours after surgery. Bruising is also common, especially if you had extractions along with implant placement. Mild discomfort is expected, but it should be manageable with prescribed or over-the-counter pain medication.
What to Do
Apply ice packs to the outside of your face for 20 minutes at a time during the first 24 to 48 hours. This reduces swelling. Take your medications as directed and don’t skip doses, even if you feel fine. Rest with your head elevated to minimize swelling.
What to Avoid
Don’t rinse vigorously, spit, or use a straw. These actions can dislodge the blood clots that protect your surgical sites. Avoid hot foods and drinks, smoking, and alcohol because they interfere with healing.
What to Eat
Stick to liquids and soft foods. Smoothies, yogurt, applesauce, and soups are good choices. Make sure everything is lukewarm or cool before you eat it. Hot temperatures can irritate the surgical sites and even cause damage to healing tissue.
Week One: Establishing Your Routine
By the end of the first week, swelling should be going down. You should be feeling more like yourself.
Oral Hygiene
After 24 hours, you can start gentle rinsing with warm salt water. Do this several times a day, especially after meals. Dr. Doroshenko will show you how to clean around your temporary teeth without disturbing the implants.
Don’t brush directly over the surgical sites yet. Use a soft-bristled toothbrush and gentle motions on the rest of your mouth. Avoid alcohol-based mouthwashes because they can irritate healing tissue.
Diet
You can start adding softer solid foods if you feel comfortable. Scrambled eggs, mashed potatoes, and soft pasta are good options. Continue avoiding hard, crunchy, or sticky foods. Your temporary teeth aren’t designed for heavy chewing.
Activity
You can return to work within a few days if you feel up to it. Don’t do strenuous exercise for the first week. Elevating your heart rate can increase swelling and bleeding.
Warning Signs
Call our Owings Mills dental office if you have severe pain that doesn’t improve with medication, bleeding that won’t stop, swelling that gets worse instead of better, or a fever. These could indicate a problem that needs attention.
Weeks Two to Four: Adjusting to Your New Smile
By the second week, most patients feel back to normal. Swelling is gone, discomfort is minimal, and you’re adjusting to your temporary teeth.
Oral Hygiene
You can resume more normal brushing around the temporary teeth. Dr. Doroshenko will show you how to clean underneath the prosthesis. Food and bacteria can get trapped between the temporary teeth and your gums, so you really need to pay attention. If you don’t clean it properly, this can lead to infection.
Use a soft-bristled toothbrush and non-abrasive toothpaste. Consider using an interdental brush or water flosser to clean hard-to-reach areas.
Diet
You can expand your diet, but continue avoiding very hard or sticky foods. Your temporary teeth are functional but not as strong as your final restoration will be.
Follow-Up Appointments
Dr. Doroshenko will schedule follow-up visits to monitor your healing. He’ll check the implant sites, evaluate your bite, and make any necessary adjustments to your temporary teeth. These appointments are important, so don’t skip them.
Months Two to Six: Osseointegration
This is the phase where the magic happens. Osseointegration is the process where your jawbone fuses with the implant posts. It typically takes three to six months.
What to Expect
You won’t feel anything happening. But beneath the surface, your bone is growing around the implants, creating a permanent foundation. This is what makes All-on-4 so stable and long-lasting.
Oral Hygiene
Continue your daily routine. Brush, floss, and clean around the temporary teeth. Dr. Doroshenko may recommend specific products to keep your gums healthy during this phase.
Diet
You can eat most foods, but be mindful of your temporary teeth. Avoid biting very hard foods like apples or crusty bread. Cut food into smaller pieces and chew on both sides of your mouth.
What’s Next
Once osseointegration is complete, Dr. Doroshenko will attach your final restoration. Dr. Lazer designs these final teeth to match your facial features, skin tone, and goals. This is the smile you’ll have for the rest of your life.
Long-Term Care: Protecting Your Investment
Once your final restoration is in place, you’re in maintenance mode. Here’s how to protect your investment for decades.
Daily Oral Hygiene
Brush twice a day with a soft-bristled toothbrush and non-abrasive toothpaste. Clean around the implants and underneath the prosthesis. Dr. Doroshenko will show you the best techniques for your specific restoration.
Water flossers are excellent for cleaning around All-on-4 restorations. They reach areas that traditional floss can’t. Interdental brushes are also helpful.
Regular Dental Visits
Visit us every six months for professional cleanings and checkups. Dr. Doroshenko will examine your implants, check your bite, and look for any signs of problems. Early detection helps prevent complications. We’ll also take X-rays periodically to monitor bone levels around your implants.
Avoid Bad Habits
Don’t chew on ice, pens, or your fingernails. Don’t use your teeth as tools. These habits can damage your restoration and stress your implants.
If you grind your teeth, tell Dr. Doroshenko. He can create a custom night guard to protect your investment while you sleep.
Watch for Warning Signs
Peri-implantitis is the most serious threat to your implants. It’s an infection that affects the gums and bone around the implants. Warning signs include bleeding gums, swelling, pain, and a feeling that the implant is loose.
If you notice any of these signs, call our office immediately. Early treatment is much more successful than waiting.
